Microscopic endodontics
This is the treatment of root canals using a large magnification, which is provided by an operating microscope. The root canals of a healthy tooth contain pulp. This is formed by connective tissue and blood vessels that nourish the tooth with the necessary nutrients. The pulp also contains nerve fibres that inform us if something is wrong with the tooth.
A typical example is inflammation of the dental pulp. It manifests itself as a strong and long-lasting pain that forces you to seek medical help. The cause of the pain is inside the tooth. Irreversible damage to the pulp has occurred, which is why it needs to be removed before the infection spreads from the tooth to the surrounding bone.
It is important to realise that root canals are very thin structures inside the root. They may not be visible to the naked eye. If we are unable to remove all the infection from the tooth, problems may persist and the tooth will need to be removed. In other words, root canal treatment is the last chance to save the tooth, and therefore all available modern technologies must be used to ensure that the treatment is of the best possible quality.

How is root canal cleaning done?
Endodontic treatment under a microscope includes the following stages:
- ensuring optimal conditions for entry into the root canals
- mechanical and chemical processing of the pulp cavity and root canals
- filling the resulting cavity inside the root with a suitable material
Teeth that need root canal treatment are often destroyed by extensive caries. Before we start treating root canals, we first apply local anaesthesia and isolate the tooth from the oral cavity with a latex membrane (rubber dam), which ensures an absolutely dry working field. After that, we carefully remove all the tooth decay to prevent repeated infection of the “treated” canal, and only at this stage can we proceed to the actual treatment of the root part of the tooth.
By using an operating microscope, we are able to discover and pass through the root system, which would be impossible to reveal without high magnification. The basic prerequisite for endodontic treatment is the removal of all infection from the root canals. Therefore, it is necessary to measure the length of the root canals accurately using a device called an apex locator and process the canals with root instruments so that we can rinse them with a disinfectant solution. Root canal disinfection is a very important step, so we pay due attention to it. The solution in the dental canal is activated by an ultrasonic device in order to increase its effectiveness.
Then we dry the canals and use the Elements free system to perform a three-dimensional filling of the root system with warm gutta-percha.
What do you need to think about after root canal treatment?
The tooth may be sensitive shortly after endodontic treatment. The sensitivity usually resolves itself within a few days and the patient is completely free of problems. A much more serious problem is increased fragility, because the tooth has lost the nutrition provided by the dental pulp. It has thus become more prone to the formation of dangerous cracks. Therefore, it is always necessary to protect the tooth with some type of dental replacement after root canal treatment.
